Pass through connection mode options to sqlite

Support custom field names in json queries

Instead of using the first part of the key directly, respect
defined custom field names

JSON: Cast key when using the equality operator

- Perform file existence checks using `fs.open` for older versions of node.

- Default to SQLite's default of OPEN_READWRITE | OPEN_CREATE
- Added tests to verify access errors when connection opened with a more limiting mode

Fixed `operator does not exist` exception when using queries like:
